SARATOGA SPA STATE PARK’S 2013 LEARN-TO-SWIM PROGRAM - 07/08/2013

Saratoga Spa State Park will be holding its annual Learn-to-Swim program from July 8th through July 19th, 2013 at the Victoria Pool.

Who:                Any child between the ages of 5 and 12  

Where:            VictoriaPool

When:              Monday – Friday, July 8 – 19, 2013, between 8a.m.and 10a.m.

Cost:                $45 per student.

 How:                One registration session remaining (at theSaratogaSpaStateParkAdministrationBuilding):

¨        Saturday, June 15^th, from 12p.m. – 2p.m.

Bring:               1. Payment (cash or check)

2. Medical information

¨        Child’s guardian’s name, address and phone number

¨        Child’s doctor’s name, address and phone number

¨        Child’s insurance information (name of company and ID number)

¨        List of any allergies or special conditions

¨        Date of last tetanus shot

¨        List of medications being taken
